Original, signed Dominican Republic passport with at least 6 months of remaining validity.
Passport Photo: 1 Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a surcharge associated with this service.
Proof of Status. Original GNIB card or other proof of resident status in Ireland, this should be valid for more than 6 months after your return from Malaysia.
Itinerary. Copy of round trip tickets or confirmed itinerary.
Yellow Fever Vaccination.
If the applicant has traveled within the last 90 days to any of the Yellow Fever Countries
an International Certificate of Vaccination for Yellow Fever will be required.
Bank Statement. Copies of original bank statements from the past 3 months. The closing balance showing on bank statements must be more than £600.
Hotel Reservations. Copy of confirmed hotel reservations including name of the applicant, name and address of the hotel, duration of stay.
Personal Invitation. If visiting friends or family, you must provide letter of invitation with the contact information of the host and visitor, who is responsible for the expenses, purpose and duration of the visit, confirmation of accommodation including the address, signature and date. You will also need to provide proof of the host's status in Malaysia ie. copy of their Malaysia passport's information page, or, if they are not a citizen of Malaysia, copies of their Malaysia residence permit and their national passport's information pages.
Employment Letter. Employment Letter. Copy of a letter from your employer on business letterhead, with contact details, purpose and duration of the trip and that you will be returning to your current job. If you are self-employed, include a copy of your business license and tax return. If you are retired please submit proof of your retirement fund.
